Get started with an intake appointment
We offer a range of programs and services that are trauma-informed and evidenced-based, and that are designed to respond to each person’s situation.
Everyone is welcome to access our services, and our psychotherapy is self-referral, which means you do not need a referral from a healthcare or support services professional. Each person starts with an intake appointment, either in person (in the location of your choice) or virtually.
To get started, please request an intake appointment. You will need to set up an account with OnCall Health, including a password, to request the appointment. You will submit one online form when you request the appointment, and then two more forms once your account has been created. These forms must be completed before your appointment or it will be cancelled as we have a high demand for our services.
If you do not have email, or are unable to complete the online booking due to the accessibility limitations of our site, please contact reception at 905-725-3513, ext 110.
We usually have a waitlist for our services, and the length of wait is dependent on your situation. Once you have completed your intake session, we will be able to give you a clearer picture of how long you will be waiting for services.
Consent forms
We have a number of consent forms for our services. Our staff will let you know what forms are required to participate in each service.
- Client Rights and Privileges
- Consent For Receiving Services by Telephone or Video Conferencing
- Use of Email Consent Form
- Parental Consent Form (For children under age 12)
- Safer Families Project Participation Agreement
Please note that we are not a crisis service. If you are in crisis and cannot wait for a therapy appointment, please call the Distress Centre at 905-430-2522 or go to your nearest hospital. Additional crisis support organizations can be found here.
Session confidentiality
All information discussed during your therapy sessions will be kept in the strictest confidence unless written permission is given by you to release such information to other agencies, professionals, or the courts. Exceptions may be made in life-threatening situations or where child abuse or neglect is suspected or present. Agencies are also subject as a matter of law, to a subpoena, and so in some circumstances a client record or a clinical therapist’s evidence may be required by a court of law.
Paying for our services
We are a low-cost, fee-for-service agency. The fee is set with you based on circumstances, ranging from $0-80. Your therapist will discuss the fee schedule with you during the first interview and the two of you will arrive at a mutually agreed upon fee.
We ask that you pay your fee after each session if at all possible. No one is turned away because of the fee.
Our services are not covered by OHIP, however many group benefits packages will cover psychotherapy. Please consult your benefit booklet to see if your coverage extends to RSWs (Registered Social Workers) and/or RPs (Registered Psychotherapists).
Cancelling your appointment
There are many people waiting for our services. We ask you to be responsible about keeping your appointments. Cancellation of scheduled appointments must be made at least 24 hours in advance. Otherwise, except where unusual circumstances have made that impossible, you will be charged one-half of the set fee. If two appointments are missed without adequate notice, the agency reserves the right to terminate your therapy for the time being. You must re-apply for therapy and you will be offered an appointment when your name comes up on the waiting list.
Requesting your report
The agency requires that clients provide a notice of five business days when requesting letters, reports, or similar correspondence. Any request for written reports beyond basic attendance at a program will be charged at the rate of $100.00 per page. This may be prorated on a sliding scale consistent with the agency’s sliding fee schedule.
Supporting children under the age of 12
It is an agency expectation that parents or legal guardians remain on agency premises while minors (under 12) are at the agency for therapy sessions. Therapists may require parental/legal guardian input or support at any time during the session and parents/legal guardians are to be available to children as needed. In exceptional circumstances where this may not be possible, any parent or legal guardian who chooses to leave a child on site without a guardian present is required to sign a waiver releasing the agency of any liability while their child is on the premises.
